In order to set up your membership for online and print marketing, we need some additional information.

Please complete these details as soon as possible. It may take several days to "go live", so act quickly to ensure that you are getting the full benefits of membership from day 1.

Your W-9 (see below for details)

Please read the instructions on the W-9 form carefully. If you are a sole-proprietor or even an LLC that is a disregarded entity, in most cases you should complete the form with your Social Security Number NOT your Federal Tax ID Number or Employer Identification Number. AND PLEASE do not use BOTH your SSN and EIN on the form. If you are unsure how to complete the W9, please seek the advice of your accountant.
