Member Profile Update

Please use this form to change or update any of your membership information.

The deadline for changes to be effective the following month (including withdrawals) is the 15th of the current month. Any changes submitted after the 15th will take place in the subsequent month (up to 46 days after submission).

Changes in Tiers or Withdrawals can only take place at the end of a member’s contracted term and only if the change/withdrawal is submitted by the 15th of the final month of the term using this form.

If you are changing your membership tier, please see the document called Changing Membership Tiers for instructions about what information is needed to process your request.

If you are simply updating information, (after the company name & membership tier) you only need to complete the information that you would like changed. You can disregard all other fields.

    Online Store Showcase(Required)
    This VIP Perk requires some training on how to add products/services to the store. I understand that 1) I need to schedule time with the Orchestrated Events website administrator (scheduled through the OE office) in order to utilize this Perk, 2) I will be trained on how to utilize the app to add/remove/update. products/services to the OE online store, 3) I will be fully responsible for adding/removing products/services and keeping all related information current and accurate.

    Also, please read the instructions on the form carefully. If you are a sole-proprietor or even an LLC that is a disregarded entity, in most cases you should complete the form with your Social Security Number NOT your Federal Tax ID Number or Employer Identification Number. AND PLEASE do not use BOTH your SSN and EIN on the form. If you are unsure how to complete the W9, please seek the advice of your accountant.

    Be aware withdrawals can only be made at the completion of a term. If you are unsure when your term ends, please check the OE Google Calendar.

    Also, you must submit your withdrawal request no later than the 15th day of the month prior to the end of your current term. In other words, if your term ends June 30th, and you want to withdraw, you must submit this form by June 15th. Any withdrawal requests submitted after this date would be effective at the end of the next term (if your term is 3 months, your withdrawal would be effective after September 30th.)
